The Missouri State High School Activities Association (MSHSAA) is now requiring any individual that will be participating in Band to have a valid sports physical on file before the marching season begins. Please make sure to get an appointment scheduled today with your primary care provider to get a physical scheduled before the end of the summer.
Provided in the links below is information regarding activity / sports physicals. The first link is a copy of the Pre-Participation Physical Evaluation Form (valid for 2 years) and the second link is the MSHSAA Pre-Participation Documentation - Annual Requirements.
The only document that requires seeing a doctor is the Pre-Participation Physical Evaluation Form. All others may be completed at home.
If your student had a 2-year physical completed last year, all that would need to be filled out this year would be the MSHSAA Pre-Participation Documentation - Annual Requirements Form.
These same forms are to be used if a student is wanting to participate in any sports next school year.

A valid and up-to-date copy of each of these forms must be on file with the District prior to participation.
